2009 Membership Re-Application is available below.  You must have either MIcrosoft Word or Adobe Reader  to access this file. 

2009 ANFRC Application

There are two application periods each year.  The deadline dates, effective May 2009, are July 31 and January 31 of each year.

For current sites, the "evidence" period is for  October 2007 through September 2008. 

For new applicants, the "evidence" period is for  the 12 months prior to the application, unless otherwise specified.
